Mr. Smith lives at 400 E 3rd Street, Duluth, MN. The two most common patient identifiers are full name and date of birth. These are the FIRST things to write on a prescription. This way you dont write a signed prescription without a patient name on it that accidentally falls out of your white coat and onto the floor in the cafeteria.

Learning the system for writing prescriptions requires actually writing them and reviewing the above guidelines to be sure the instructions and format are correct. Use your resources to assure accuracy. When unsure, check your drug reference literature on how to write a prescription, especially for an infrequently used medication. Latisse is a drug that requires a physician prescription.
Prior to obtaining Latisse its important to have an appropriate consultation with the prescribing physician. At the time of consultation you will be evaluated with a history and physical examination. Badly handwritten prescriptions can lead to mistakes, and it is the legal duty of the doctor to write legibly (Box 7).
In prescriptions for controlled drugs or those with a potential for abuse it is safer to write the strength and total amount in words, to prevent tampering.
